As dental professionals we know what you expect from a network and we are committed to serving your needs. MeritDent would like to welcome you to become a participating provider of our PPO network. Listed below you will find just a few of the benefits you will enjoy by participating in the MeritDent network:

The process is simple, just follow the steps below:

  1. Select the forms listed below:
  1. Print and fill out the forms (use the checklist to help you keep track of what you need to send us)
  2. Fax to (702) 966-3760 or mail back the completed forms (phone and fax number are listed on the forms)

You must have the credentialing information to us by the 15th of each month for you to be effective the 1st of the following month. Please complete one packet for each provider per office.
